Teams must register together to play in a FUTSAL EAST league. Individual registration forms will NOT be accepted. Each team must have an adult coach/manager, and an adult on the sideline at each game. To register, complete the attached team registration form and collect the attached player waiver forms for each player you will roster. NOTE: You do not need to send in the player release forms with your team registration...you can bring them to your first game. The 08/09 FUTSAL EAST REGISTRATION FORM / PLAYER RELEASE will be available in September 2008.
NOTE: All of our sessions filled before the registration deadlines last year, so register early. |
